Shoebox Pick-Up Booking Form - minimum 80 shoeboxes

THE SHOEBOX APPEAL FOR THIS YEAR IS NOW CLOSED. THANK YOU TO EVERYONE THAT PARTICIPATED.   

 Next register for a pick up  using the form and then follow the instructions below:-

  • Complete the Delivery Note.
    You should have received this in the post after you registered for the Appeal. Alternatively, go to Shoebox Downloads (see left) to print your own copy.
  • Collect the Gift Aid forms and £2 coins from the Shoeboxes. 
    Sending a cheque for the total amount is really helpful. 
And put it all together in an envelope clearly marked with your,
  • Name
  • Group name
  • County
  • Postcode.
Our drivers will collect your envelope, along with all the Shoeboxes, and then we can begin the work of bringing smiles to impoverished communities in Eastern Europe.

Please would you have your Delivery Note, Gift Aid Forms and Money (or cheque for total amount) in an envelope marked with your Name, Group Name, County and Postcode, and ready to give to the driver. This paperwork is essential for us to accurately record your involvement in the appeal, and to plan for next year’s appeal.
